Aims: By the end of the leson, Ss will be able to: - know the organization of content of a form - know how to fill in a form B. Procedure: Time Activities 4’ 7’ 7’ 5’ 10’ 10’ 2’ Warm-up : - Show and give some kinds of form - Let students practise answering the questions: 1. Have you ever seen a form? 2. Have you filled/ used a form? 3. When did you use it? - Lead in the new lesson I. Pre-writing - Elicit the new words: - form (n) mẫu (đơn) - fill in điền vào nationality (n) quốc tịch marital status (n) tình trạng hôn nhân block capital (n) chữ viết hoa - surname (n) họ - Read aloud all the word. - Read again and ask Ss to repeat after T. - Call on some Ss to read aloud. * Checking: What and Where II. While-writing 1. Task 1 (task 2 in textbook) - Explain what information to make a form - Ask for information - Let students read quickly the column A and B - Ask students to match individually then compare in pairs, groups - Call on Ss to give the answers -> Answer keys 1. d 2. f 3. b 4. g 5. b 6. c 7. a - Give feedback. - Let each pair ask in A and answer in B 2. Task 2 (task 3 in textbook) - Ask students to do certain things - It means we use short information but it’s real - Ask students to read the model of form first - Explain some new words: + delete (v): unnecessary to use or write out + male: for man + female: for woman - Let students write on the board or in the papers - Go around and remark 3. Task 3 (task 4 in textbook) - Ask students to fill some information in a form - Explain how to fill in the form - Ask 2 or 3 students who may be good at English to write on the board - Let students change their forms and practice speaking about the person they’ve written - Listen and give feedback III. Post-writing: - Divide class into 4 groups - Let students work in groups to write a form about someone and other groups - Write a short paragraph. - Call on the leaders of 4 groups to go to the board and show their products. - Give feedbacks IV. Homework: - Review the lesson - Do the exercise in workbook - Prepare the next lesson
